MyChoice activities are open to all Advocates CSS participants

MyChoice is a unique service that falls under the category of “Agency with Choice or AWC.” MyChoice is based on the philosophy of Self-Determination. Every individual has an individualized plan that is facilitated by a broker. The broker helps the individual and the family/friends develop a customized plan that reflects the individual’s unique goals and objectives. The individual plan outlines the activities, schedule, and supports that the person needs to successfully achieve their outcomes. All of this is the same as Self-Determination with a few additional features that Advocates has developed.

MyChoice is a collaborative model where brokers and coordinators assist the individual and their family with finding staff, managing staff, scheduling back up staff and sharing staff in the community. In addition, MyChoice plays a proactive role by providing a MyChoice Coordinator who helps individuals identify community opportunities for participation (work, volunteer, social, educational). The funding is allocated to the individual through the Fiscal Management Service just like it is with self determination. The broker may play an enhanced role in helping the individual and family manage the budget and allocate the resources per their request.

MyChoice is Self-Determination with a lot of coaching and collaboration from Advocates. This allows the person to achieve their individualized objectives through using the Self-Determination philosophy, without the isolation and total “do it on your own” aspects of traditional Self-Determination.
